Surgeons Choice Medical Center offers safe and secure access to your personal medical records, anytime day or night. This free service will help you become more informed and prepared to better understand your healthcare options.  In addition, you have the ability to make payments to your healthcare account using the Patient Portal link on this page. If you have any issues logging into the patient portal please call your toll-free MyCareCorner Support Hotline Number: 1-855-478-8101 (9am - 6pm EST) for assistance.

In order for you to access your information or make payments online, you will need to provide us with an email address. This email address will be used only to send pertinent information about creating a login and password to allow access to your electronic health information through our patient portal MyCareCorner.  If you provided your email address upon patient check-in but did not receive the link sent to your email address within 48 hours, please call 248-423-5170 to speak with a Medical Records Representative.  If you do not have an email address printed instructions with a unique access code will be provided to you to self-register for the patient portal.  You may also designate an authorized representative whose email address may be used to register for your patient portal.   

Once you have set up your email address and password, or received your printed access code, you will be able to register for the Patient Portal.  If you received your code via email, click the link provided and the access code will automatically fill in the registration page.  If you received your access code via printed instructions, you would need to manually enter the access code to register for your patient portal.

Once logged in, you have the ability to make a payment. To begin, click Payment Center (which will only appear if you have a balance).  Next, you can choose to "Pay Now" to pay the balance in full or choose "Patient Payment Options" to set up a payment plan and choose the amount for payment.  Finally, complete the Payment Method screen to enter your payment information.  Once payment is complete, you will have the option to email, text, or print your receipt.

If you would like a copy of your medical record, you can choose to print it from within your patient portal or request a paper copy of your health information, click on the link Request My Medical Records on this page.  A document will download to your computer for you to print and complete.  Once completed and signed, you can either mail or fax the form to the address/fax number in the top right-hand corner of the form.  As an alternative, you may bring the completed form to Surgeons Choice Medical Center. Once arrived, please ask security for Medical Records and someone will greet you at the security desk.
