Insurance types accepted at Surgeons Choice Medical Center

Surgeons Choice has established arrangements with many different types of health care insurers.  The below list is not all inclusive of all the health plans with which Surgeons Choice is contracted.

• Medicare
• Medicaid
• Priority Health
• Tricare
• HMOs
• Other Commercial Insurance companies
• Other insurance types such as disability insurance, motor vehicle insurance and workers' compensation

Your benefit coverage for care provided by Surgeons Choice is determined solely by your insurance company and is based on the provisions of your specific medical benefit plan.  Member benefits vary between plans, so it is recommended you follow your plan's rules and speak to your health insurance representative.  Please contact the customer service department on the back of your health insurance member identification card to confirm that you have in-network access to Surgeons Choice as well as your physician.
